We operate using a simplified and affordable membership based model called Direct Primary Care (DPC). DPC is an alternative path to accessing healthcare that doesn’t involve insurance.
This model allows us to spend more time with you while keeping cost low, removes unnecessary layers between you and your provider, and offers transparent pricing that eliminates co-pays and other surprise after-visit fees.

$75 per month per person (couples discount available). One affordable and predictable monthly fee regardless of the number of times you need care. No copays or surprise after visit fees.

Low cost labs. We negotiated with Labcorp for awesome savings and pass these savings on to you. Members receive a set of labs free each year, check it out HERE

No long term contracts, 30 days written notice needed to cancel prior to the next billing period.

KATLYN AGOSTA, MSN, APRN, FNP-C, CDCES
Nurse Practitioner, Diabetes Care and Education Specialist

Katlyn Agosta is board-certified autonomous family nurse practitioner and certified diabetes care & education specialist. She lives in Valrico with her husband, their two children, a dog and backyard chickens.
Katlyn's diagnosis of type 1 diabetes guided her to the field of nursing and is what fuels her passion for diabetes management and education. She has firsthand experience with insulin pump therapy and continuous glucose monitoring systems since 2002.
The ability to connect on a personal level, laugh with her patients, educate and support individuals on their healthcare journey is why she fell in love with primary care.
Katlyn loves reading fiction, gardening (though her plant life track record is questionable), painting, and playing video games with her family. You can find her Friday mornings working out with the FiA Fishhawk group.
